The Professional Regulation Commission (PRC) informs the public that the four (4) pieces of unused ROR Slips of Optometrists (New Law) bearing serial numbers 22-0124417 to 22-0124420 were lost and beyond recovery. Diligent search and effort to recover the accountable forms were made but to no avail.

Any transaction involving the said accountable forms shall not be honored or entertained in any PRC Offices or Service Centers.

The public is encouraged to report to the PRC any encountered transactions involving the subject ROR Slips either in person at any PRC Offices nationwide or through email at arta@prc.gov.ph or rating@prc.gov.ph.
